4.  Copy of all applicable State and Federal permits, or applications for same, if still under review (e.g. KDOW Floodplain Permit, Water Quality Certification,

5.  Surety Bond Information (contact name, address, fax, phone)


I certify that the foregoing information is true and correct to the best of my knowledge and belief, that I fully understand the provisions of the City of Pikeville Erosion and Sediment Control Ordinance and that I accept responsibility for carrying out the Erosion and Sediment Control Plan for the above referenced project as approved by the City.

I certify that all land clearing, construction, and development involving the movement of earth shall be in accordance with the Erosion and Sediment Control Plan approved by the City, and that I will provide a qualified representative on site on all days when construction or grading activity takes place.

I understand that the issuance of this permit in no way guarantees the approval of any rezoning, special use permit, building permit, subdivision plat, site plan, or any other plan of development.

I understand that the City requires an erosion and sediment control bond and an application fee as follows.  The Application Fee must be submitted with this application.  The bond will be required before work may commence on the approved Erosion and Sediment Control Plan.  If this work is not completed according to the approved Plan, and within the approved time frame, the City has the right to draw upon the erosion and sediment control bond in order to complete the work.

                        Base Fee - Per amount of Acreage Disturbed

                        No more than 1 Acre                                                               $100.00

                        Each additional acre or portion there of                                  $  25.00/acre

I further understand if construction is not executed within one (1) year of the issuance of the Erosion and Sediment Control Permit, the permit will be void and fees will not be refunded.

I hereby grant the City of Pikeville the right to enter upon the subject property periodically to ensure compliance with the aforesaid ordinance.
